Wiring advice for new home

This is a graphic showing the present setup of my network in my old home. All wiring is external and the modem, Juniper, router, switch are all in the same location as the various networked devices (including the phones). I am building a new home. Data cabling will be wired in the walls. Since I need to be able to connect the 2 VOIP phones as well as the router to the Juniper (which is used for voluntary emergency medical service dispatching through the VOIP phones), it seems that I will need to have 3 separate cables running from 3 jacks to the Juniper, correct? The problem arises is that I may be installing a 3rd VOIP phone in a different location which will also need to connect to the Juniper. It seems that I will then need a 4th cable & jack for this one. Am I correct or am I missing something? I am far from a pro at networking and the tech support for the volunteer dispatch organization is minimal and mainly after-the fact, so I am mostly on my own making sure to get it right. I need all the help I can get to do this correctly.
